Tell us about your practice and confirm eligibility. We’ll personally confirm by email or phone before the case is submitted.

2) Submit Your Case

Send a digital scan (preferred) via TRIOS, Medit, or DS Core, or ship a traditional impression with a completed Rx.

3) Receive Your Crown

Standard turnaround applies. We follow your Rx closely and keep you posted if anything needs clarification.
